RE: Door Closure Policy
It has come to the attention of this department that expenditures for repair and maintenance of door hardware are far above the budget allowance. Investigation has revealed that this is due to excessive operation of doors and their associated hardware. We have set up a program to address this problem.
Doors that are open are to remain open, doors that are closed are to remain closed.
If, for any reason, the state of a door is required to be changed, a DOOR STATE CHANGE REQUEST FORM (form1283 rev6/96) must be filled out and approved by your departmental door state supervisor, or in his absence, your departmental door state manager prior to changing the state of the door. If your departmental door state supervisor and manager are unavailable, forms should be submitted to the divisional door state administrator for approval.
A separate request must be submitted for each change of state of a door. For example, if you are requesting to close a conference room door for a meeting, you must submit separate forms for closing the door at the start of the meeting and opening the door at the end of the meeting.
Company policy dictates that in the event of a fire, doors to the affected area are to be closed immediately. If this situation should arise, you must fill out and submit an EMERGENCY DOOR STATE ALTERATION FORM (form 1463 rev7/96). A separate form must be submitted for each door. DO NOT submit a DOOR STATE CHANGE REQUEST FORM. If it is subsequently determined that the door state was altered due to a false alarm, an ERRONEOUS ASSUMPTIVE ACTION NOTIFICATION FORM (form 1196 rev5/96) must be submitted by the individual closing the door. If the state of a door is altered by outside emergency personnel, it is the responsibility of the departmental door state supervisor to fill out and submit the required forms.
All forms required by this directive are available from the internal publications department and should be requested on a FORM REQUEST FORM (form 1001 rev5/96) with costs assigned to the requesting departments operating budget.
